Thermage heats the dermis with monopolar radiofrequency while cooling the surface, so contraction happens below skin that is protected. It works on skin quality and tightness across a broad field rather than on a focal vector, which is why it is often staged with ultrasound lifting rather than chosen instead of it.

The eye tip runs at separate settings for periorbital skin, which is thinner and does not tolerate face parameters.
